Open SafetyLog EHS

Safety shouldn't cost a fortune

Free, open-source incident tracking for manufacturing. Report accidents and near misses, investigate them, follow corrective actions through to done, and keep your OSHA records.

About the Project

The Overview dashboard for the Northstar Co. workspace: counts of open incidents, actions due this week, overdue actions and average time to close, a table of recent incidents with type, severity, location and status, and bars showing how many incidents are at each workflow stage.

Environment, health and safety (EHS) software is usually priced per seat and sold to large companies. Smaller plants end up tracking injuries in spreadsheets and paper binders, which makes it hard to see trends, follow up on fixes, or fill out OSHA forms at the end of the year. Open SafetyLog EHS gives those plants the same core tools for free, and they can run it on their own server.

It's in early development. Reporting, investigations, corrective actions, OSHA recordkeeping, analytics and email notifications all work end to end.

What It Does

  • Shop-floor reporting. Anyone can report an incident or near miss from a shared kiosk tablet or a printed QR code, without an account. Reports made offline are sent when the connection returns, and the app works in English and Spanish.
  • Investigations and corrective actions. Safety managers investigate each incident, assign corrective actions with due dates, and get reminders until they're closed.
  • OSHA recordkeeping. A short set of questions decides whether a case is recordable under 29 CFR 1904, flags injuries that must be reported within 8 or 24 hours, and builds Forms 300, 300A and 301 per site, plus the files for OSHA's electronic submission.
  • Analytics. Leading and lagging indicators, including TRIR and DART rates, with saved views.

The Incidents register: filters for type, severity, status and site, saved views and CSV export above a list of incidents such as a forklift near miss, a wet floor hazard and a hand laceration, each with its location and workflow status.
